FOOD, NUTRITION, AND PHYSICAL ACTIVITY FOR PEOPLE WITH MEMORY CHANGES AND THEIR CAREPARTNERS

Are you interested in exercise and nutrition? Investigators from the University of Waterloo are interested in what you have to say about your exercise and nutrition needs. A Food, Nutrition, and Physical Activity survey is being conducted to design wellness programs for older adults with memory changes and their family/friend care partners. If you are a community-dwelling older adult with memory changes, or a care partner to a community-dwelling older adult with memory changes, then you are eligible for the study. You may complete the survey online at www.bit.ly/nutritionexercise survey .

The survey is expected to take 10-15 minutes of your time. Alternatively, please contact researchers Heather Keller (hkeller@uwaterloo.ca) or Laura Middleton (lmiddlet@uwaterloo.ca) to receive a mailed or telephone version of the survey. French version available upon request. This study has been reviewed and received ethics clearance through a University of Waterloo Research Ethics Committee.
