The Passport has been fully revised to bring all its information up to date. As before, the Passport takes the participant on a healthy lifestyle journey of a six-week workshop. The Passport primarily focuses on Type 2 diabetes but also covers the relationship between healthy lifestyle choices and other chronic diseases.

The Passport is designed to provide participants with benefits that will last a lifetime. It will allow the participant to go on a journey of opportunities and new experiences. In the process, they will learn how physical activity, food choices, lifestyle habits and maintaining positive mental health influence several chronic diseases.

Lifestyle habits are difficult to change. Small steps and simple changes are encouraged. The Passport is designed to be a personal tool to motivate and educate older adults. It is also to be used to assess the participant’s lifestyle, record their activities and successes along the road to healthier living.

  • Increased exercise and knowledge
  • Improved eating habits, weight loss and smaller waist measurement
  • Stable blood sugars
  • Decrease in blood pressure
  • Greater self-confidence and motivation to continue

Updated Tool Kit Resource for Healthy Living Workshops

The new updated Tool Kit is designed for use by a non-clinical community leader, and provides a step by step guide to offer a six-week healthy living workshop for older adults. The Tool Kit provides resources for the leader to help people who want to start, or maintain, healthier lifestyles. It also now includes a chapter to help the leader manage an online virtual workshop.

The workshop is based on the most current research available. Dedicated experts worked with Active Aging Canada to develop, test, and validate the content. The workshop is designed to guide the leader through the use of Your Personal Passport to Healthy Living for the participants. The Tool Kit provides:

  • Background information on the development and use of the Tool Kit
  • A new chapter that explains how to conduct virtual workshops online
  • Step by step guide for the six-week course
    • Getting Started
    • Motivating Change
    • Healthy Eating
    • Physical Activity
    • Mental & Emotional Health
    • Sustaining Healthy Lifestyles & the Impact of Chronic Disease
  • Activities for each module
  • Online videos, resources, handouts and promotional posters for the workshops
  • Inclusion of the Community Leader’s Guidebook

The lesson plans provide tips on how to use the Passport and are holistic in their approach. The Tool Kit is designed to support healthy active aging for both men and women, supports self-management of chronic disease, and incorporates the Canadian 24-Hour Movement Guidelines for Adults (65+ years).

The past ten years have confirmed that the Passport will help to inform, educate and inspire behaviour change in your participants.

The Guidebook is designed to provide the group leader with additional resources, workshop tips, background information, and tools and techniques for effective evaluation of your workshop.

While the Passport is designed for use by the older adults in your programs, the Guidebook will assist you in facilitating the use of the Passport, providing you with:

  • Information on the risk factors, signs and symptoms, and prevention of Type 2 diabetes
  • Ethics of using the Passport with individuals and groups;
  • Insight on motivating behaviour change in older adults
  • Tips on facilitating groups in using the Passport
  • Suggestions on how to measure participant progress and the success of your program
  • Guidelines around the successful use of their Personal Passport to Healthy Living
  • Resource and reference materials

In June 2021, virtual facilitation training was provided to community leaders to provide them with the skills and knowledge to offer online, our Healthy Living Workshop for Older Adults.

This workshop utilizes Your Personal Passport to Healthy Living as the main resource for the participants. The Passport was also revised in 2021, and can be ordered online.
